Minor MyChart "Proxy" Access
MyChart access for a minor is done via proxy access to a parent or a guardian.
It is simple, the parent or guardian requests access and will subsequently have access from within the parent/guardian account. After you request access, medical records approves the request and access is granted. The reply says it can take up to 3 business days, but generally it is done same or next day as the request.
If parent/guardian is not a patient of the practice, please call John Muir Health Patient Support Center @ (925) 941- 2001 and they will assist you in setting up an account to then setup proxy access.
Login to MyChart:
To login, either click the link in the upper right hand corner of this website, or go to https://www.johnmuirhealth.com and click on MyChart Login in the upper right hand corner:
This will take you to the login page:
Once logged in, click on "Your Menu" icon in the upper left:
Scroll down to the bottom of the list, under "Account Settings", and click "Personalize":
Click on "Request access to a minor's record":
Fill out all fields, make sure to certify by clicking the "I certify..." box and click on "Submit Request":
You will then see a confirmation message that your request has been submitted.
